To place an order for pre-mixed music cds, you have several options:

Option 1- Online order and payment
To order online and pay online using your credit card, go to our Music CD page and click the "Add to Shopping Cart" Button next to the CD's you want to order. The shipping charge is $15.00

Option 2- Print This Form and Fax with payment information

Fill out the form below, and then print to your local printer. Do not leave any information blank.
